Putnam County Breast Health Project

Due to a grant from the Northwest Ohio Affiliate of Susan G. Komen for the Cure to fight breast cancer in Putnam County, the Putnam County Health Department is offering a program to help low to middle income women access mammograms and diagnostic breast ultrasounds for the purpose of screening for breast cancer.

The eligibility criteria for financial assistance to pay for a mammogram through the project include:

Household income of no more than 300% of the federal poverty guidelines (See guidelines below).

Determined by the income from any adult living in the home and contributing to household expenses

Private health insurance with an individual deductible of at least $200 or no insurance.

bullet

Between 40 and 64 years old.

bullet

If aged 30-39 years, must be at a high risk for breast cancer because of family history or personal history of breast disease or abnormality.

After receiving a mammogram, those enrolled in the program will receive a gas gift certificate to Putnam County Shell stations for travel expenses.  

If you or someone you know meets the criteria, please complete and return the information sheet (infosheet.pdf ) and/or call the Putnam County Health Department (419-523-5608) to enroll in the program and get the breast screening that could save your life.  Persons currently enrolled in Medicaid or those who have no insurance are encouraged to call and get appropriate referral sources for mammography or possibly enroll in the program also.
